When I create a new page or edit a page, the horizontal category menu bar disappears. Is that the intended functionality? My users are finding it a bit disorienting to not see it. Or is there a setting I need to change?
Thank you for your question. When a page is edited, it will always have the look of default Confluence. So if you are referring to the surroundings of the page, it is intentional that it is not shown.

On my Confluence 6.6.1, the default theme look is for the menu bars to remain visible even when editing. So it seems like you are saying contradictory things. But if it is the default look of Refined Wiki to remove those bars, I guess we will learn to live with it, unless there is a way to bring the menu bar back through the global stylesheet?
Yes, I can see how it would seem to be contradictory. The bar on top of the page when editing it is actually part of the default theme, so when a page is edited in default theme, then the bar is shown. However, for RefinedTheme we have chosen to not show any of the theme when a page is being edited, and that is why the bar is not there.
I hope this makes more sense now, even though I do understand your point of view when working with default themes and RefinedThemes at the same time. I have checked with the dev team and bringing it out is not a simple matter, unfortunately.
